All patients with an HMO policy must have either Dr. Sung Park-Davis or Dr. Heather Koza selected as their PCP in order to be seen.

Yes. If your insurance is an HMO insurance, all specialist visits require authorization from your primary care physician (PCP). Certain tests and procedures may require prior authorization. Please contact our office if you believe you need authorization for any scheduled services. You must see a provider at CIHC before making appointment with any specialists.
Once you discuss your request for a referral or authorization with your PCP it will be processed within 7 business days, unless we need to contact you for further information. We will fax the referral authorization directly to the specialist or facility. In certain instances, it may be accessible online based on your insurance. You may also contact the specialist or facility to confirm receipt.
Yes, referrals are valid for 90 days from the date they are issued, which typically aligns with the date of your referral request. This allows sufficient time for you to schedule an appointment with the specialist.
Schedule an appointment with your PCP when you require a specialist/testing facility. Our processing time is 7 business days. Failure to notify our office in advance may result in out-of-pocket expenses for your visit or test.
